Chaperone/Staff Paperwork

Thank you for your interest in volunteering with us! We can always use extra help at camps, rehearsals, concerts, and when we travel. Please download the volunteer packet below and follow the steps to go through the process. Paperwork stays on file for 3 years and is good for any FCS school. Paperwork can be turned into the PHS Front Office or Band Office with a copy of your drivers license.

Level 2

This level is for volunteers who do NOT want to ride the bus with us at away games, competitions, and trips.

Clearance Requirements 

▪ Volunteer Release Form 

▪ State-issued photo identification required

▪ Georgia sex offender registry through Raptor

▪ Clearance requirements: GA criminal background check through PSI

▪ Confidentiality and Mandated Reporter Agreement 

▪ Approval from the principal or designee 

Level 3 

This level is for all instructors or clinicians that are brought in and any volunteers that want to ride the bus with us to away games, competitions, or any other trips (including the Spring Trip). This level required a state and federal background check through PSI (fingerprinting) and costs around $50. 

Clearance Requirements 

▪ Volunteer Release Form 

▪ State-issued photo identification required

▪ Georgia sex offender registry through Raptor 

▪ Clearance requirements: Federal & GA criminal background through GAPS

▪ Confidentiality and Mandated Reporter Agreement

▪ Cogent fingerprinting on file with HR

▪ Approval from HR and principal or designee required
